Fixing that issue you seem to have produced a now one:
In the cart the custom price for the chosen option is multiplied with the product amount.

One additional issue:
In the select box options in product view you do not display a custom price, if it is 0 - that is good.
But in the cart you display a custom zero price.
Please also in the cart display the custom price, if it is non zero, only.
Or alternatively wrap a zero price in a span element having a class "zeroprice" for example; so the developer can decide whether to display a zero custom price, or not.

We didn't change any of the code for the calculation of the custom prices. It's by design it will multiple on more products.
For instance: you order a sandwhich and as an extra you want double-cheese for € 0,50 extra. Later in the order process your friend also wants the same sandwich - so you change the quantity from 1 to 2. Isn't it logical the custom price is doubled - in this case?
That's how the custom field is intended from within VirtueMart and this ReqText plugin.
We've put the zero-price-span-wrap request on our feature-request list.

I understand your point of view.
There is just one problem:
It is confusing for customers.
Why?
In the VM cart the usual way to display product positions is:
product information per item (i. e. name, qualities) | price per item (containing surcharges per item) | quantity | position discount | position price
And: When the customer saw in the product information in detail view a surcharge of i. e. 10.00 for a quality, displaying 20.00 in the product information in the cart is confusing.
Is this somehow comprehensible?

Could you, please, include in your brainstorming an option to choose, whether the custom prices should be displayed at all?
